Understanding Dandruff and Scalp Barrier Health

Flaking, itching, and the difference between dryness and dermatitis.

Dr. Rahul Iyer · MD, Dermatology · Chennai

Not all flaking is the same

Simple dryness, seborrhoeic dermatitis and psoriasis all present with flaking and are managed differently. Treating one as another is the most common self-care mistake I see.

If flaking comes with redness, persistent itching or spread beyond the scalp, it needs assessment rather than a change of shampoo.

What the barrier does

The scalp has the same barrier function as the rest of the skin, and it can be compromised by over-washing, harsh surfactants and aggressive scratching.

Restoring comfort is usually a matter of doing less rather than more.

Where oil fits, and where it does not

A light oil, applied gently and washed out reasonably, is well tolerated by most people and can ease the sensation of tightness.

In active seborrhoeic dermatitis, heavy oiling can worsen matters. That is a case for a clinician, not a ritual.
